| 目的：基于咽肾关系理论探究宣清和化方治疗IgA肾病的应用价值。方法：选取2019年6月至2021年8月武汉市中医医院收治的IgA肾病患者70例作为研究对象，采用简单随机化随机分为对照组和观察组，每组35例，对照组采用常规治疗，观察组在对照组基础上加用宣清和化方，2组患者均治疗24周。比较2组患者临床疗效、中医证候积分、血清IgA、尿红细胞计数、24 h尿蛋白定量（24 h-UTP）、血肌酐（Scr）、血尿素氮（BUN）、血尿酸（UA）、估算肾小球滤过率（eGFR）、超敏C反应蛋白（hs-CRP）、γ干扰素（IFN-γ）、肿瘤坏死因子-α（TNF-α）、白细胞介素-2（IL-2）、IL-4、IL-5、IL-6、IL-10及药物不良反应。结果：治疗后24周，观察组临床疗效优于对照组（P<0.05），观察组中医证候积分低于对照组（P<0.05）。观察组血清IgA、尿红细胞计数、24 h-UTP、Scr、BUN、UA低于对照组（P<0.05），eGFR高于对照组（P<0.05）。观察组hs-CRP、TNF-α低于对照组（P<0.05），IL-4、IL-5、IL-6、IL-10高于对照组（P<0.05）。2组一般不良反应差异无统计学意义（P>0.05）。结论：基于咽肾关系理论使用宣清和化方辅助西药治疗IgA肾病可进一步提高疗效，减轻临床症状，改善肾功能和炎症水平，安全性良好。 |

| To explore the application value of Xuanqing Hehua Recipe in treating immunoglobulin A(IgA) nephropathy based on the theory of pharynx-kidney relationship.Methods:A total of 70 patients with IgA nephropathy who were admitted to Wuhan Hospital of Traditional Chinese Medicine from June 2019 to August 2021 were selected and divided into the control group(35 cases，conventional treatment) and the observation group(35 cases，Xuanqing Hehua Recipe on the basis of conventional treatment) by simple randomization method.After 24 weeks of treatment，the clinical efficacy，traditional Chinese medicine(TCM) symptom scores，serum IgA，urine red blood cell count，24 h urine total protein(24 h UTP)，serum creatinine(Scr)，blood urea nitrogen(BUN)，uric acid(UA)，estimated glomerular filtration rate(eGFR)，high-sensitivity C-reactive protein(hs-CRP)，interferon-gamma(IFN-γ)，tumor necrosis factor-α(TNF-α)，interleukin-2(IL-2)，IL-4，IL-5，IL-6，IL-10 and adverse drug reactions were compared between the groups.Results:After 24 weeks of treatment，the clinical effect in the observation group was better and the TCM symptom scores were lower than those in the control group(both P<0.05).The observation group had decreased serum IgA，urine red blood cell count，24 h UTP，Scr，BUN，UA，hs-CRP and TNF-α while increased eGFR，IL-4，IL-5，IL-6 and IL-10 compared with the conditions in the control group(all P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion:The use of Xuanqing Hehua Recipe combined with western medicine based on the theory of pharynx-kidney relationship can further enhance the clinical efficacy，relieve clinical symptoms and improve the renal function and inflammation of patients with IgA nephropathy，with high safety. |
