| 目的：分析玉屏风散化裁联合沙美特罗替卡松雾化吸入治疗慢性阻塞性肺疾病（COPD）稳定期患者的疗效。方法：选取2020年3月至2021年7月琼海市中医院收治的稳定期COPD患者86例作为研究对象。采用简单随机化随机分为对照组和观察组，每组43例。对照组给予沙美特罗替卡松雾化吸入粉雾剂治疗，观察组在对照组基础上联合玉屏风散化裁治疗，2组均治疗12周。比较2组临床疗效和不良反应发生情况。比较2组治疗前后中医证候积分、呼气流量峰值（PEF）、第1秒用力呼气容积（FEV1）、用力肺活量（FVC）、FEV1/FVC、气道壁厚度、气道内外径、气道腔面积、外周血CD4+、CD3+、CD8+水平、CD4+/CD8+比值、血清基质金属蛋白酶-2（MMP-2）、MMP-3、MMP-9、基质金属蛋白酶抑制剂（TIMP）1、TIMP2、白细胞介素-6（IL-6）、C反应蛋白（CRP）、降钙素原（PCT）水平。结果：观察组总有效率高于对照组（P<0.05）。治疗后，观察组各中医证候积分、血清MMP-2、MMP-3、MMP-9、CD8+、IL-6、PCT、CRP水平及气道壁厚度和气道腔面积低于对照组，FEV1、FVC、FEV1/FVC、TIMP-1、TIMP-2、CD4+、CD3+、CD4+/CD8+水平及气道内直径高于对照组（P<0.05）。2组总不良反应发生率比较，差异无统计学意义（P>0.05）。结论：玉屏风散化裁联合沙美特罗替卡松雾化吸入治疗稳定期COPD患者疗效明确，改善免疫功能，降低炎症反应，增强肺功能，有效抑制气道重塑。 |

| To analyze the curative effect of modified Yupingfeng Powder combined with seretide inhalation on patients with stable chronic obstructive pulmonary disease(COPD).Methods:A total of 86 patients with stable COPD treated in Qionghai Hospital of Traditional Chinese Medicine from March 2020 to July 2021 were enrolled.According to the simple randomization method，they were divided into control group(n=43) and observation group(n=43).The control group received seretide inhalation，while the observation group was treated with modified Yupingfeng Powder on the basis of the control group.After treatment lasted for 12 weeks，the clinical curative effect and adverse reactions of the two groups were compared.The traditional Chinese medicine(TCM) syndrome scores，peak expiratory flow(PEF)，forced expiratory volume in one second(FEV1)，forced vital capacity(FVC)，FEV1/FVC，airway wall thickness，airway medial-lateral diameter，airway lumen area，levels of CD4+，CD3+ and CD8+，CD4+/CD8+ in peripheral blood and levels of serum matrix metalloproteinase-2(MMP-2)，MMP-3，MMP-9，tissue inhibitor of metalloproteinase(TIMP) 1，TIMP2，interleukin-6(IL-6)，C-reactive protein(CRP) and procalcitonin(PCT) before and after treatment were compared between the two groups.Results:The total response rate of observation group was higher than that of control group(P<0.05).After treatment，the TCM syndrome scores，levels of serum MMP-2，MMP-3，MMP-9，CD8+，IL-6，PCT and CRP，airway wall thickness and airway lumen area in observation group were lower than those in control group，while the levels of FEV1，FVC，FEV1/FVC，TIMP-1，TIMP-2，CD4+，CD3+，CD4+/CD8+ and airway medial-lateral diameter were higher than those in control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Modified Yupingfeng Powder combined with seretide inhalation has significant curative effect on patients with stable COPD，which can enhance immune function，reduce inflammatory response，improve pulmonary function and inhibit airway remodeling. |
